How do you know when someone has ADD or ADHD?

ADD and ADHD are very difficult diagnoses to make. Diseases are only diseases if there's an actual problem. Not being able to sit still isn't actually problem in iteslf. (And isn't remotely sufficient to diagnose ADD or ADHD).
